==== Ionic selectivity and plasticity ==== Several groups reinvestigated the permeability properties of TPCs and their role in NAADP-induced Ca2+ release, and they agreed that TPCs are indeed permeable to Na+ but they could not necessarily recapitulate the Na+ selectivity shown in the 2012/13 studies. It was therefore initially proposed that TPCs may conduct both Ca2+ and Na+ (analogous to the NMDA receptor of the plasma membrane). As more studies were published, why some groups observe a Na+ selectivity while others see a mixed Na+/Ca2+ permeability was unclear until the important realization that the TPC2 ionic selectivity wholly depended on the activating ligand. Currents activated by PI(3,5)P2 were predominantly carried by Na+ whereas NAADP-activated currents showed an eight-fold increase in the Ca2+ permeability. This conveniently explained the discrepancies between groups as well as revealing that TPC2 is extraordinarily plastic in operating in different conductance modalities. Since then, it appears that TPC2 can be synergistically activated by co-application of NAADP and PI(3,5)P2, although the molecular mechanisms are unclear. Therefore, TPC2 can operate as either a Ca2+ or Na+ channel, depending on whether NAADP or lipid activate it.

Opicapone, sold under the brand name Ongentys, is a medication which is administered together with levodopa in people with Parkinson's disease. Opicapone is a catechol-O-methyltransferase (COMT) inhibitor. The most common side effects are dyskinesia (difficulty controlling movement), constipation, increased blood creatine kinase, hypotension/syncope, and decreased weight. Opicapone, works to restore the levels of dopamine in the parts of the brain that control movement and coordination. It enhances the effects of levodopa, a precursor of the neurotransmitter dopamine that can be taken by mouth. Opicapone blocks an enzyme that breaks down levodopa in the body called catechol-O-methyltransferase (COMT). It is peripherally selective and does not inhibit COMT in the brain. As a result of its COMT inhibition, levodopa remains active for longer. This helps to improve the symptoms of Parkinson's disease, such as stiffness and slowness of movement. In June 2016, it was authorised for use in the European Union. It was authorised for use in the United States in April 2020.

The therapeutic index (TI) is used to quantify the risks and benefits of a certain drug. It describes the relationship between toxic dose and minimum effective dose, thus providing an important insight into the safety of a drug. The Therapeutic Index is calculated using the following equation: TI = TD50 / ED50, where TD50 is the dose at which toxicity presents in 50% of the population, and ED50 is the dose needed to produce 50% of maximal response. From the equation, a high TI indicates that the drug needs a high dose to induce toxicity in 50% of the population or a low dose to achieve the minimum effective dose, and vice versa. In the case of physiological antagonists, for example, insulin has a narrow TI. A narrow TI indicates that either excess or lack of insulin can cause significant risks. On one hand, lack of insulin may result in high blood glucose levels and kidney or cardiovascular damage. On the other hand, excess insulin may result in insulin-induced hypoglycemia as aforementioned. Another example is dimercaprol, a chemical antagonist in treating metal toxicity. Dimercaprol has a narrow TI so it is replaced by its derivative, 2,3-dimercaptosuccinic acid (DMSA).

Mice treated with SynthoPlate exhibited significantly reduced bleeding times following tail transection compared to controls, with effects observed within a two-hour circulation period. Immunostaining and immunoblotting confirmed enhanced fibrin formation at the injury site, indicating that SynthoPlate effectively supports clot formation in platelet-deficient conditions.
=== Strategies for toxicity prevention === Understanding the mechanisms behind the toxicity of gliotoxin can open new possibilities for the use of gliotoxin therapeutically or as a diagnostic test for some conditions. One potential strategy that has been explored to reduce the toxicity of the fungi that produce gliotoxin is to target the gli gene cluster that controls the expression of gliotoxin protein. The disulfide bridge of gliotoxin is crucial to its toxicity, so it is theorized that the tailoring of enzymes to prevent the disulfide bridge closure by interfering with GliT or by catalyzing another reaction to block the sulfur residues may be beneficial in reducing the toxicity of those fungi. Another potential strategy is the targeting of the transcriptional activator GliZ, as deletion of the GliZ resulted in abrogated gliotoxin biosynthesis. This leads to the possible targeting of GliZ itself rather than any gene-based methodology to prevent it from binding to the gli gene cluster and activate transcription of the genes required for gliotoxin biosynthesis. One possible strategy for disrupting the regulation of gliotoxin transport is depleting the amount of GipA in the cell. GipA is a transcriptional regulator for the expression of the GliA transporter protein, which is required for gliotoxin secretion. These biosynthetic strategies for reducing the toxicity of pathogenic fungal strains that produce gliotoxin are still in their early stages of exploration but could provide novel methodologies for the adoption of therapeutic uses for gliotoxin.
