=== Degradation === Once an insulin molecule has docked onto the receptor and effected its action, it may be released back into the extracellular environment, or it may be degraded by the cell. The two primary sites for insulin clearance are the liver and the kidney. The liver clears most insulin during first-pass transit, whereas the kidney clears most of the insulin in systemic circulation. Degradation normally involves endocytosis of the insulin-receptor complex, then the action of insulin-degrading enzyme on the B-chain, followed by the disulfide bond being broken by enzymes from the protein disulfide isomerase family. An insulin molecule produced endogenously by the beta cells is estimated to be degraded within about one hour after its initial release into circulation (insulin half-life ~ 4–6 minutes).
Repeated heroin use changes the physical structure and physiology of the brain, creating long-term imbalances in neuronal and hormonal systems that are not easily reversed. Studies have shown some deterioration of the brain's white matter due to heroin use, which may affect decision-making abilities, the ability to regulate behavior, and responses to stressful situations. Heroin also produces profound degrees of tolerance and physical dependence. Tolerance occurs when more and more of the drug is required to achieve the same effects. === Doping in sport === SARMs including enobosarm may be and have been used by athletes to assist in training and increase physical stamina and fitness, potentially producing effects similar to anabolic steroids. For this reason, SARMs were banned by the World Anti-Doping Agency in January 2008, despite no drugs from this class yet being in clinical use, and blood tests for all known SARMs have been developed. There are a variety of known cases of doping in sports with enobosarm by professional athletes.
The expression of hepcidin, which only occurs in certain cell types such as hepatocytes, is tightly controlled at the transcriptional level and it represents the link between cellular and systemic iron homeostasis due to hepcidin's role as "gatekeeper" of iron release from enterocytes into the rest of the body. Erythroblasts produce erythroferrone, a hormone which inhibits hepcidin and so increases the availability of iron needed for hemoglobin synthesis.
Evidence is insufficient to support the use of medications to treat obstructive sleep apnea directly. This includes the use of fluoxetine, paroxetine, acetazolamide, and tryptophan among others. Recent studies are trying to investigate cannabinoids as a treatment for OSA, especially dronabinol, which is a synthetic form of THC (delta-9-tetrahydrocannabinol). Cannabis is known to influence sleep; for example, it can reduce sleep latency. However, results are not consistent. Studies about dronabinol have shown positive impact on the OSA, as they observed a reduced AHI (Apnea-Hypopnea Index) and an increased self-reported sleepiness (the objective sleepiness being unaffected). However, more evidence are needed as many effects of those substances remain unknown, especially the effects of a long-term intake. The effects on sleepiness and weight gain are particularly of concern. Due to uncertainty about its effects and a lack of consistent evidence, medical cannabis is not recommended for the treatment of OSA. There are multiple drugs approved for managing the excessive daytime sleepiness associated with OSA, but not the underlying cause. These include solriamfetol, modafinil, and armodafinil. For adults with obesity and moderate-to-severe OSA, during a 52-week study tirzepatide (brand name Zepbound) substantially reduced apneic-hypopnic events, body weight, hypoxic burden, hsCRP concentration, and systolic blood pressure, and improved sleep-related patient-reported outcomes, all presumably mediated via induced weight loss of 18–20% from baseline.
== Cardiology == In cardiology, genetic conditions such as Brugada syndrome can share features with related disorders caused by mutations in the same gene. An overlap syndrome can be seen whereby a mutation in the SCN5A gene encoding the cardiac sodium channel causes a reduction in the peak sodium current leading to the typical ECG features of Brugada syndrome, but which simultaneously increases the sustained late sodium current leading to the ECG features of Long QT syndrome type 3. Brugada syndrome can also overlap with arrhythmogenic cardiomyopathy due to certain mutations in the plakophilin gene.

=== Scope of the national home "in Palestine" === The statement that such a homeland would be found "in Palestine" rather than "of Palestine" was also deliberate. The proposed draft of the declaration contained in Rothschild's 12 July letter to Balfour referred to the principle "that Palestine should be reconstituted as the National Home of the Jewish people." In the final text, following Lord Milner's amendment, the word "reconstituted" was removed and the word "that" was replaced with "in". This text thereby avoided committing the entirety of Palestine as the National Home of the Jewish people, resulting in controversy in future years over the intended scope, especially the Revisionist Zionism sector, which claimed entirety of Mandatory Palestine and Emirate of Transjordan as Jewish Homeland This was clarified by the 1922 Churchill White Paper, which wrote that "the terms of the declaration referred to do not contemplate that Palestine as a whole should be converted into a Jewish National Home, but that such a Home should be founded 'in Palestine.'" The declaration did not include any geographical boundaries for Palestine. Following the end of the war, three documents – the declaration, the McMahon–Hussein correspondence and the Sykes–Picot Agreement – became the basis for the negotiations to set the boundaries of Palestine.
Through a process called melanogenesis, melanocytes produce melanin, which is a pigment found in the skin, eyes, hair, nasal cavity, and inner ear. This melanogenesis leads to a long-lasting pigmentation, which is in contrast to the pigmentation that originates from oxidation of already-existing melanin. There are both basal and activated levels of melanogenesis; in general, lighter-skinned people have low basal levels of melanogenesis. Exposure to UV-B radiation causes increased melanogenesis. The purpose of melanogenesis is to protect the hypodermis, the layer under the skin, from damage by UV radiation. The color of the melanin is black, allowing it to absorb a majority of the UV light and block it from passing through the epidermis. Since the action spectrum of sunburn and melanogenesis are virtually identical, they are assumed to be induced by the same mechanism. The agreement of the action spectrum with the absorption spectrum of DNA points towards the formation of cyclobutane pyrimidine dimers (CPDs) - direct DNA damage. Developments in Britain helped to alienate the white Rhodesian population from the former mother country, namely Queen Elizabeth II's 1966 speech to the Parliament of Jamaica where she endorsed majority rule and her attempt in 1968 to block the hanging of what Rhodesians perceived as Zimbabwean nationalist terrorists. Until late 1969, Rhodesia still recognised Queen Elizabeth II as head of state, even though it opposed the British government itself for hindering its goals of independence. However she refused to accept the title Queen of Rhodesia and eventually the Smith government abandoned their attempts to remain loyal to the British Crown. In a 1969 referendum, a majority of the electorate voted to declare Rhodesia an independent republic. The hope being that this move would facilitate recognition as an independent state by the international community, but the issue of white minority rule remained and continued to hinder this effort, and like the UDI before it, the proclamation of a republic lacked international recognition. The British public overall viewed the issue of Rhodesia and UDI as a distant and anachronistic one that they cared relatively little about and saw any expedient response to it by their government as satisfactory. Most Britons saw the end of Rhodesia's white supremacist timocracy as inevitable and justified. Although the majority Britons opposed military intervention against the renegade Rhodesian state, this opinion was not uniformly held.
